On this day in Australian aviation history:

1942 Lockheed Hudson Mk I A16-173 operated by 32 Squadron RAAF crashed at Dapto, Wollongong, NSW on 4 November 1942. The Hudson took off from Camden at 1645hrs to carry out a enemy submarine search some 300 miles east of Sydney. It went off course when returning and in darkness, rain, and low cloud it flew into the terrain (Bong Bong Mountain) near the Avon Colliery at Dapto. Sources: ADF-Serials; aviation-safety.net website
1999 The Deputy Prime Minister and Minister for Transport and Regional Services, The Hon. John Anderson MP, released a policy statement "A Measured Approach to Aviation Safety Reform" on 4 November 1999, along with new charter letters for AirServices Australia and CASA, following the Hawke report. The statement indicated that airspace design, declaration and management would remain with ASA while CASA would retain responsibility for minimum airspace standards and operating procedures. The Minister also sought resolution of experimental and sports aviation, education and ATC matters. Meanwhile ASA settled in secret the TAAATS contract dispute with Hughes Corporation. 2010 Qantas Airbus A380-843 VH-OQA "Nancy Bird Walton" operating Qantas Flight 32 suffered an uncontained failure in one of its four Rolls-Royce Trent 900 engines on 4 November 2010 as it climbed towards cruising altitude above the Riau Islands, Indonesia. The aircraft was operating a regularly scheduled passenger flight from London to Sydney via Singapore and the failure occurred four minutes after takeoff from Singapore Changi Airport. After holding in the air for almost two hours to assess the situation, the aircraft made a successful emergency landing at Changi. No injuries occurred to the passengers, crew, or people on the ground, despite debris from the aircraft falling onto houses in Batam.
